Part C is an Advantage Plan (HMO or PPO), are plans offered by private companies that are approved by Medicare. This plan will include all of Part A and Part B, it does also has options to be able to have extra's on the plans.
Part D helps cover the cost of prescription drugs (including many recommended shots or vaccines).
